As soon as my children start reading independently, I have them fill out this simple book report form.
In the younger years, I just have them draw in the boxes so it’s not too intimidating. Once they have mastered the literature terms, I require them to write in complete sentences. I switch off between this form and one I created a few years ago.
It’s always FUN for the children to complete a creative book project too.
You can even use this form for read a louds, too.
